Financial Disclosure Forms

District employees and public officials who are required to file Financial Disclosure Statements should use the following forms provided by BEGA.

Partial Waiver Request Form Partial Waiver Request Form 

Public officials required to make their PFDS reports publicly available should use this form to submit a request to BEGA to consider whether there is good cause to exempt the public official from some or all of the public filing requirement.

E-filing Waiver Request Form E-filing Waiver Request Form 

FDS filers may use this form to request a waiver of the requirement to file their FDS forms electronically.

Policy Deadline Extension Request Form 

FDS filers seeking an extension of up to thirty (30) days to submit their FDS forms must submit this form to the Director of Government Ethics before the FDS filing deadline

Designation Appeal Form Designation Appeal Form 

Form for use by employees seeking to challenge their designation as a Financial Disclosure Statement Filer.

Fine Waiver Request Form Fine Waiver Request Form 

Public and Confidential FDS filers should use this form to submit a request to BEGA to consider whether there is good cause to waive all or a portion of any assessed late or non-filer fees.
